I was wondering if any company has ever made a AC output accessory transformer. Sorta like a transformer that ya plug in and switch on and puts out 16 volts of AC power. Has one ever been made?

They have been made, not for model railroading, but will work for accessories anyway!A local Electrical Surplus Center, will have alot of them and at a place like that are decent in price.

Well...yeah...ever heard of LIONEL ? 0 - 18 volts of AC power, strictly for model railroading. Why not use one of those for the accessories ? Also, old telephony equipment used 18 VAC for their relays, matter of fact I just plucked a box of office supplies out of the Dumpster Depot behind my building today, and in there was a 5vdc power supply, and a Brothers 18 VAC power supply for a fax or something similar. a couple of volts won't make a huge difference, might shorten the life a tad. But if you use an old Lionel transformer, you can cut down the voltage to 15, 14, etc.
Short answer, yes there are VAC transformers/power bricks out there.

The output of all transformers is AC, which has to be rectified in later stages for use with DC motors and electronics. An AC power pack is much simpler, and as Jerry stated was the standard for years with Lionel, Flyer, etc. before solid state diodes were developed.
A lot of guys use old wall warts or train set transformers to power accessories. They don't have to be AC, either.
