They must have attempted transport between two starships traveling at warp speed. It IS possible, although Commander Scott hasn't written the equation yet

Probably left in direct sunlight, maybe on a window sill. A hobby shop owner discovered that they couldn't display models in their display windows because the greenhouse effect would melt the merchandise.
